At Fountains Family Care, we understand that facing a serious or life-limiting illness can be overwhelming for both patients and their families. Our hospice consultation services are designed to provide compassionate guidance, education, and support during difficult healthcare decisions.
A hospice consultation helps patients and families better understand available care options, goals of care, symptom management, and whether hospice services may be appropriate. Our team takes the time to carefully review the patient’s medical condition, functional decline, overall prognosis, and personal wishes to help families make informed and comfortable decisions.
During a hospice consultation, we may discuss:
- Eligibility for hospice care
- Goals of care and advance care planning
- Pain and symptom management
- Emotional and family support needs
- Home care and caregiver resources
- Quality of life considerations
- Coordination with hospice agencies and specialists
Dr. Richard Le has extensive experience in hospice and palliative care and serves as Medical Director for multiple hospice organizations throughout Arizona. His compassionate approach focuses on preserving dignity, comfort, and quality of life while supporting both patients and their loved ones through every stage of care.
Our goal is to ensure that patients receive the right level of care, support, and comfort while helping families feel informed, supported, and never alone during the process.
